OVERVIEW

All passport applicants, whether for new passports or renewals, must appear in person at the Philippine Consulate General in Frankfurt. An appointment is required for each individual applicant. Bring all the requirements, as incomplete applications will not be processed.

Passport appointments are made via the passport.gov.ph website.

Key Information for Applicants

  •  Lost Passport: Immediately report a lost passport to the Consulate by sending an email to frankfurtpcg.consular@dfa.gov.ph.
  • Processing Time: New passports may take 4 to 6 weeks to be available from the date of filing of application. However, the Consulate is not responsible for unexpected delays in the delivery of the passport.
  • Passport Delivery: Applicants are required to provide a prepaid envelope with €4.45 worth of stamps from Deutsche Post (Einschreiben) or from a private courier service (e.g. DHL). The Consulate assumes no liability for any loss of documents once they have been handed over to the German Post Office/courier service.
  • Unclaimed Passports: Passports not claimed within one year will be canceled per DFA Department Order No. 2021-012.
  • Passport Validity: Airlines and immigration authorities require at least 6 months’ validity of  your passport. The Consulate strongly recommends applying for a new passport 9 months before your current one expires and advises against purchasing travel tickets until you have your new passport.

BASIC REQUIREMENTS FOR ADULT NEW PASSPORT

Supporting documents

Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship and absence of travel restrictions, as specified in the following cases:

In case of Late Registered PSA-issued Birth Certificate, Report of Birth, or Certificate of Foundling:
  • Original and photocopy of PSA-issued Birth Certificate, Report of Birth, or Certificate of Foundling
  • Additional valid primary government-issued ID accepted for passport application;
  • If unable to provide an additional primary ID, the applicant must submit any two (2) of the following documents:
    • NBI Clearance (valid or expired);
    • School Records such as:
      • Form 137-A
      • Transcript of Records
      • Diploma obtained from Elementary, High School and/or College
    • If government employee, Service Record;
    • Member Data Record (MDR) from PhilHealth
If the Passport applicant’s PSA-issued Birth Certificate, Report of Birth, or Certificate of Foundling lacks data, or the first name is registered as “Baby Boy/Baby/BabyGirl/Girl/Boy” and the applicant is born before 1993:

Applicant must submit a PSA-issued Birth Certificate, Report of Birth, or Certificate of Foundling with the proper annotation.

If the passport applicant’s PSA-issued Birth Certificate, Report of Birth, or Certificate of Foundling contains a misspelled first or last name, or misspelled birth place, or mistake in the day or month of birth, or clerical error in the sex, or change of first name or nickname:

Applicant must submit a PSA-issued Birth Certificate, Report of Birth, or Certificate of Foundling pursuant to R.A. No. 9048, as amended by R.A. No. 10172 with the proper annotation.

If by operation of law or through a court order, the passport applicant is permitted to use a name or other biographical details other than those officially recorded in the PSA:

Applicant must submit a PSA-issued Birth Certificate, Report of Birth, or Certificate of Foundling reflecting the corrected entry/ies.

Discrepancies on the biographical details of supporting documents submitted for passport application:
Applicant shall provide supporting documents that are consistent with the information as stated in the applicant’s PSA-issued document, unless by operation of law or through a court order, the applicant is permitted to use a name other than what is officially recorded in the PSA. In case of inconsistency of entries between a PSA-issued document and the supporting document, the applicant shall have the supporting document corrected for its details to be consistent with the PSA-issued document. The applicant shall submit the following depending on the correction needed:
  1. PSA-annotated Birth Certificate
  2. PSA-annotated Report of Birth
  3. PSA-annotated Certificate of Foundling
  4. PSA-annotated Marriage Certificate
  5. Corrected Government-issued Valid IDs
  6. Corrected Identification Certification
If the PSA-issued documents submitted by the applicant is unreadable:
  • For unreadable PSA-issued Birth Certificate (COLB) or Certificate of Foundling: Local Civil Registry Municipal Form No. 102 or Civil Registry Form 1-A; If born abroad, a copy of the Report of Birth is required.
  • For unreadable PSA-issued Certificate of Marriage: Local Civil Registry Municipal Form No. 97 If married abroad, a copy of the Report of Marriage is required.
  • For unreadable PSA-issued Certificate of Death: Local Civil Registry Municipal Form No. 103
In case applicant has NO PSA-issued Birth Certificate or Report of Birth:
  • Applicant must first file for late registration with Local Civil Registrar (LCR) or Consular Office with jurisdiction over the place where applicant was born; and
  • Applicant must submit a PSA-issued late registered Birth Certificate or Report of Birth.
If applicant is a naturalized Filipino citizen:
  • For persons who have undergone Administrative Naturalization under Republic Act No. 9139:
    • Certificate of Naturalization;
    • Certificate of Renunciation of foreign citizenship issued by the concerned foreign Embassy or foreign Ministry; and
    • Identification Certificate issued by BI.
  • For persons who have undergone Judicial Naturalization under Commonwealth Act No. 473:
    • Certificate of Naturalization;
    • Court Order, Certificate of Finality of Decision granting naturalization, Decree of Naturalization; and
    • Identification Certificate issued by BI.
  • For persons who have undergone Legislative Naturalization under Commonwealth Act No. 63:
    • Certificate of Naturalization;
    • Certified true copy of the law granting citizenship;
    • Foreign Birth Certificate authenticated by the Philippine Foreign Service Post; and
    • Identification Certificate issued by BI.
  • For persons who availed themselves of the Facilitated Naturalization of Refugees and Stateless Persons under Supreme Court Rule 21-07-2022-SC:
    • Decree of Naturalization
If applicant obtained Filipino citizenship by Election:
  • Applicants born before 17 January 1973 to Filipino mothers and non-Filipino fathers, and elected Philippine citizenship within three (3) years upon reaching the age of twenty-one (21) need to submit the following additional requirements:
    • PSA-issued Birth Certificate or a PSA-authenticated Report of Birth; and
    • Additional documents proving election of Philippine citizenship.
If applicant retained or re-acquired Philippine citizenship:
  • Applicant covered by the provisions of under Republic Act No. 9225, otherwise known as the “Citizenship Retention and Reacquisition Act of 2003”, shall submit any of the following documents issued by a Philippine FSP or by the Bureau of Immigration (BI):
    • Order of Approval;
    • Oath of Allegiance; or
    • Identification Certificate or Certificate of Retention/Re-acquisition of Philippine Citizenship.
  • Applicant is a derivative beneficiary of a Filipino parent who retained or re-acquire Philippine citizenship, shall submit the following documents:
    • Order of Approval of parent or child (as applicable); and/or
    • Identification Certificate issued by a Philippine FSP or Certificate of Retention/Re-acquisition of Philippine Citizenship issued by the BI.
If a married woman applicant wishes to revert to her maiden name:
Change of name should be based on the following:
  • By virtue of death of the spouse:
    • PSA-issued Death Certificate or Report of Death (ROD) of spouse or apostilled or authenticated foreign Death Certificate of foreign spouse with English translation, if applicable;
    • PSA-issued Birth Certificate or Report of Birth; and
    • Latest-issued Philippine passport (if available).
  • By virtue of an annulment, declaration of nullity of marriage, judicially-recognized foreign divorce, and judicially-recognized divorce under Presidential Decree No.1083 (Code of Muslim Personal Laws of the Philippines):
    • PSA-issued Marriage Certificate or Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage
    • PSA-issued Birth Certificate or Report of Birth; and
    • Latest-issued Philippine passport (if available).
  • Other reasons for reversion:
    • PSA-issued Birth Certificate or Report of Birth;
    • Notarized Affidavit of Explanation that includes a request for the reversion of maiden name in the Philippine passport or travel document and stating she has not previously availed of the reversion;
    • Latest-issued Philippine passport or travel document; and
    • Any valid government-issued ID reflecting the applicant’s maiden name.

BASIC REQUIREMENTS FOR ADULT RENEWAL OF EPASSPORT

  1. Personal appearance
  2. Printout of appointment confirmation
  3. Computer-generated passport  application form attached to the appointment email confirmation
  4. Current ePassport with photocopy of data page
  5. Original PSA-issued documents that will support the change of name:
    • Marriage Certificate
    • Annotated Birth Certificate
    • Annotated Marriage Certificate to show annulment/divorce/court-ordered instruction; and/or
    • Death Certificate of Spouse

In case of Lost EXPIRED NON-ePassport:
  • Police Report in English (original and photocopy)
  • Notarized Affidavit of Loss in English (may be executed at the Consulate)
  • Photocopy of the applicant’s lost passport
  • Penalty Fee: €150.00

Note: If the applicant is unable to submit a photocopy of his/her lost passport, he/she will be required to submit a photocopy of his/her PSA-issued Birth Certificate, and his/her application will be treated as a new one. Additional supporting documents may be required, as necessary, to ascertain citizenship, identity and absence of travel restrictions.

In case of renewal of a frequently lost Passport:
  • Police Report in English (original and photocopy)
  • Notarized Affidavit of Loss in English
  • Photocopy of the applicant’s lost passport (may be executed at the Consulate)
  • Penalty Fee: €150.00

Note: If the applicant is unable to submit a photocopy of his/her lost passport, he/she will be required to submit a photocopy of his/her PSA-issued Birth Certificate, and his/her application will be treated as a new one. For applicants with unclaimed passports issued in 2010, onwards:
  • Applicant may request for a Certificate of Unclaimed Passport either at the office where the unclaimed passport was issued, nearest consular office or at DFA Aseana;
  • Notarized Affidavit of Explanation; and
  • Applicant with unclaimed passport will be treated as New Applicant.
In case applicant used a travel document to travel back to the Philippines (applications in the Philippines):
  • Original Travel Document; and
  • Notarized Affidavit of Explanation
* Additional supporting documents may be required, as necessary, to ascertain citizenship, identity and absence of travel restrictions.
In case applicant lost the travel document used to travel back to the Philippines (applications in the Philippines):
  • Original Travel Records from Bureau of Immigration; and
  • Notarized Affidavit of Explanation

In case passport has been mutilated or damaged:
  • Notarized Affidavit of Explanation;
  • Photocopy of the applicant’s lost passport; and

Note: If the applicant is unable to submit a photocopy of his/her lost passport, he/she will be required to submit a photocopy of his/her PSA-issued Birth Certificate, and his/her application will be treated as a new one. Additional supporting documents may be required, as necessary, to ascertain citizenship, identity and absence of travel restrictions.

In case applicant is a Dual Citizen:
  • Natural-born Dual Citizens:
    1. Latest-issued Philippine passport; and
    2. PSA-issued Birth Certificate or PSA-issued Report of Birth.
  • Dual Citizens who availed themselves of the provisions of Republic Act No. 9225:
    1. Latest-issued Philippine passport;
    2. Original and photocopy of Oath of Allegiance, Order of Approval, or Identification Certificate issued by a Philippine FSP; or Certificate of Retention/Re-acquisition of Philippine Citizenship or any equivalent document issued by BI; and
    3. Valid government-issued ID or their secondary citizenship/residence counterparts.
In case applicant is a Naturalized Citizen:
  • Latest-issued Philippine passport; and
  • Identification Certificate of Naturalization from the Bureau of Immigration, or Certificate of Naturalization issued by the Special Committee on Naturalization (SCN), or copy of the final Court Judgment or Law granting Philippine citizenship.
In case applicant is a Filipino citizen by Election:
  • Latest-issued Philippine passport;
  • Documents of election of Philippine citizenship; and
  • Identification Certificate issued by the BI.
If a married woman applicant wishes to revert to her maiden name:

  • If born in the Philippines: The minor applicant may submit a Certified True Copy (CTC) of Local Civil Registrar (LCR)-issued Birth Certificate authenticated by the PSA.
  • If born abroad:The minor applicant may submit original copy of Report of Birth or first endorsement from the DFA’s Consular Records Division (CRD).
In case the minor applicant’s PSA-issued Birth Certificate was registered one (1) year after birth:
The applicant must submit at least one (1) of the following supporting documents:
  • School records; or
  • Baby book or health record or its equivalent document.
In case of minor applicant with married parents, as stated in the Minor Applicant’s PSA Birth Certificate:
  • Valid Philippine passport or other competent proof of identity of the accompanying parent and a copy of his/her spouse’s passport or other competent proof of identity;
  • If the accompanying parent is an alien, he/she must present his or her foreign passport and a copy of the Filipino parent’s passport or other competent proof of identity; and
  • If accompanied by an authorized adult companion, a Special Power of Attorney (SPA) executed by either of the minor’s parents or his/her legal guardian, as the case may be, his/her passport or other competent proof of identity, and a copy of the passport or other competent proof of identity of the minor’s parents.
In case of minor applicant with unmarried parents, as stated in the minor applicant’s PSA Birth Certificate:
  • The applicant’s mother shall accompany the minor applicant in the passport application process;
  • A Special Power of Attorney (SPA) executed by the mother will be required if she is not accompanying the minor applicant. The SPA must be authenticated by the Philippine Embassy/Consulate if executed abroad; and
  • Passport or valid government-issued ID of authorized adult companion
In case applicant is not accompanied by the parent/s during the application process:
  • Applicant must submit a Special Power of Attorney executed by either of the minor’s parents or his/her legal guardian designating the adult companion to assist in the application process. The Special Power of Attorney must be authenticated by the Philippine Embassy/Consulate if executed abroad;
  • Passport or valid government-issued ID of authorized adult companion; and
  • The Special Power of Attorney must be accompanied by a copy of the parent/s valid ID and/or passport.
If applicant is minor with unmarried parents, but duly acknowledged by the father and the mother is deceased:
  • Personal appearance of minor applicant and biological father;
  • PSA-issued Death Certificate or Report of Death of mother;
  • PSA Birth Certificate with Acknowledgement of Paternity (must indicate the name of the father in the birth certificate);
  • Passport or valid government-issued ID of biological father or legal guardian
In case minor is not accompanied by father:
The following may accompany the minor child in the order indicated:
  • Surviving grandparent - submit PSA-issued Death Certificate of mother;
  • Oldest brother or sister, over twenty-one years of age - submit PSA-issued Death Certificate of mother and PSA-issued Death Certificate of grandparent;
  • The child’s actual custodian (a court order for legal guardianship should be submitted)
If applicant is minor with unmarried parents, and mother is deceased / absent and father is unknown:
  • Personal appearance of minor applicant and court-appointed legal guardian
  • PSA-issued Birth Certificate
  • Valid passport or valid government-issued ID of adult guardian
  • Letter of Guardianship issued by Family Court
In case none of the above can accompany the minor applicant:
  • In default of parents or a judicially-appointed guardian, the persons exercising substitute parental authority under Article 216 of the Family Code of the Philippines shall prevail in determining the minor applicant’s companion in passport application, unless otherwise ordered by a competent Philippine court. In case of conflicting claims of custodial rights, a court order shall be required; and
  • In cases involving Filipino minors abandoned abroad, the Consular Official of the concerned Philippine FSP may initiate the passport application with the best interest of the minor being the primary consideration.
In case of Minor Foundlings in the Philippines not for Adoption:
  • Personal appearance of minor applicant and the NACC Social Worker or Liaison Officer;
  • PSA-issued Certificate of Foundling;
  • Letter of Guardianship issued by the Family Court; and
  • Valid ID of the guardian.
In case of Minor Applicant who is a Prospective Adoptee for Domestic Adoption:
  • Personal appearance of minor applicant and NACC Social Worker or Liaison Officer;
  • PSA-issued Birth Certificate or Report of Birth or Certificate of Foundling;
  • Valid passport or valid government-issued ID of authorized NACC Social Worker or Liaison Officer;
  • Certificate of Child Available for Adoption or Deed of Voluntary Commitment / Certificate Declaring a Child Legally Available for Adoption issued by the National Authority for Child Care; and
  • Official letter or Special Power of Attorney signed by the NACC Regional Office authorizing the Social Worker to process and receive the passport of the minor applicant and must include a copy of valid government-issued ID and/or passport.

Note: If an applicant is not accompanied by an authorized NACC Social Worker during the application process, the applicant must submit an SPA executed by the authorized NACC Regional Office designating the minor’s companion to assist in the application process and must include a copy of the companion’s valid government-issued ID and/or passport.

In cases of Minor Applicant who is an Adoptee through Domestic Adoption:
  • Personal appearance of minor applicant and adoptive parent/s;
  • Amended PSA-issued Birth Certificate or Report of Birth, indicating the child’s adoptive name and name of adoptive parents;
  • Passports or other competent proofs of identity of the minor applicant, adoptive parent/s or authorized adult companion; and
  • Certified True Copy of Court Decree of Adoption or NACC-issued Order of Adoption.
In case of Minor Applicant who is a Prospective Adoptee for Foreign Adoption:
  • PSA-issued Birth Certificate, Report of Birth or Certificate of Foundling;
  • Endorsement from NACC;
  • Certificate Declaring a Child Legally Available for Adoption or Deed of Voluntary Commitment;
  • Placement Authority issued by NACC;
  • Certificate for Issuance of Passport issued by NACC;
  • Travel Clearance Certificate for adoptee;
  • Child Study Report; and
  • Clearance for Intercountry Adoption.
In case of Minor Applicant who is an Adoptee through Foreign Adoption:
  • Personal Appearance of minor applicant and adoptive parent/s;
  • Amended PSA-issued Birth Certificate, Report of Birth;
  • Passport/s of adoptive parent/s or authorized adult companion. If the adoptive parents are foreign nationals, presentation of their valid foreign passports is acceptable; and

  • In cases involving Filipino minors abandoned abroad, the Consular Official of the concerned Philippine FSP may initiate the passport application with the best interest of the minor being the primary consideration.
In case of Minor Foundlings in the Philippines not for Adoption:
  1. Personal appearance of minor applicant and the NACC Social Worker or Liaison Officer;
  2. PSA-issued Certificate of Foundling;
  3. Letter of Guardianship issued by the Family Court; and
  4. Valid ID of the guardian.
In case of Minor Applicant who is a Prospective Adoptee for Domestic Adoption:
  1. Personal appearance of minor applicant and NACC Social Worker or Liaison Officer;
  2. PSA-issued Birth Certificate or Report of Birth or Certificate of Foundling;
  3. Valid passport or valid government-issued ID of authorized NACC Social Worker or Liaison Officer;
  4. Certificate of Child Available for Adoption or Deed of Voluntary Commitment / Certificate Declaring a Child Legally Available for Adoption issued by the National Authority for Child Care; and
  5. Official letter or Special Power of Attorney signed by the NACC Regional Office authorizing the Social Worker to process and receive the passport of the minor applicant and must include a copy of valid government-issued ID and/or passport; and
  6. Photocopy of the applicant's latest-issued passport.

Note: If a photocopy of the applicant’s latest-issued passport is not available, the PSA-issued Birth Certificate will be required and will be treated as a new applicant. Additional supporting documents may be required, as necessary to ascertain citizenship, identity and lack of travel restrictions.

In case of Minor Applicant who is a Prospective Adoptee through Domestic Adoption:
  1. Personal appearance of minor applicant and adoptive parent/s;
  2. Amended PSA-issued Birth Certificate or Report of Birth, indicating the child’s adoptive name and name of adoptive parents;
  3. Passports or other competent proofs of identity of the minor applicant, adoptive parent/s or authorized adult companion; and
  4. Certified True Copy of Court Decree of Adoption or NACC-issued Order of Adoption.
In case of Minor Applicant who is a Prospective Adoptee for Foreign Adoption:
  1. PSA-issued Birth Certificate, Report of Birth or Certificate of Foundling;
  2. Endorsement from NACC;
  3. Certificate Declaring a Child Legally Available for Adoption or Deed of Voluntary Commitment;
  4. Placement Authority issued by NACC;
  5. Certificate for Issuance of Passport issued by NACC;
  6. Travel Clearance Certificate for adoptee;
  7. Child Study Report; and
  8. Clearance for Intercountry Adoption.
In case of Minor Applicant who is an Adoptee through Foreign Adoption:
  1. Personal Appearance of minor applicant and adoptive parent/s;
  2. Amended PSA-issued Birth Certificate, Report of Birth;
  3. Passport/s of adoptive parent/s or authorized adult companion. If the adoptive parents are foreign nationals, presentation of their valid foreign passports is acceptable; and
  4. Certified True Copy of Court Decree of Adoption.

BASIC REQUIREMENTS FOR LOST PASSPORT

In case of a lost VALID Passport:

  • Police Report in English (original and photocopy)
  • Notarized Affidavit of Loss in English (may be executed at the Consulate)
  • Photocopy of the applicant’s lost passport
  • Penalty Fee: €150.00

Note: If the applicant is unable to submit a photocopy of his/her lost passport, he/she will be required to comply with the same requirements as for a NEW PASSPORT APPLICATION.

In case of lost EXPIRED Passport:

  • Police Report in English (original and photocopy)
  • Notarized Affidavit of Loss in English (may be executed at the Consulate)
  • Photocopy of the applicant’s lost passport
  • Penalty Fee: €150.00

Note: If the applicant is unable to submit a photocopy of his/her lost passport, he/she will be required to comply with the same requirements as for a NEW PASSPORT APPLICATION.

BASIC REQUIREMENTS FOR MUTILATED OR DAMAGED PASSPORT

In case of a MUTILATED OR DAMAGED Passport:

  • Notarized Affidavit of Explanation in English (may be executed at the Consulate)
  • Photocopy of the applicant’s damaged passport
  • Penalty Fee: €150.00

Note: If the passport is severely damaged, the applicant will be required to comply with the same requirements as for a NEW PASSPORT APPLICATION.

BASIC REQUIREMENTS FOR TRAVEL DOCUMENT

Filipinos who lose their passports may be issued a Travel Document, valid for 30 days, to allow direct, one-way travel to the Philippines. This applies to tourists, short-term students in Germany, and residents of third countries. It cannot be used for travel to any other country.

  • Duly accomplished Travel Document Application Form
  • Four (4) photos with white background (standard European passport-size photos)
  • Copy of applicant’s airline reservation / booking (please ensure your flight does not have a layover in any Schengen / EU member country)
  • Present old/expired Philippine passport, or the photocopy of the data page of the lost Philippine passport
  • Affidavit of Loss
  • Police report (with English translation by a certified translator)
  • Photocopy of birth certificate and / or marriage contract (in the case of married women)
  • Processing Fee (See updated consular fees)

REQUIREMENTS FOR PASSPORT releasing

Your new passport may be available four (4) to six (6) weeks from the date the passport application has been received by the Consulate.

Passport release can either be in person or by mail/courier.

  • For applicants who wish to receive their new passport by mail, they need to submit a self-addressed return envelope with €4.45 worth of stamps for registered mail within Germany, or a private courier service (e.g. DHL). The old passport must be sent to the Consulate by mail for cancellation.
  • For applicants who opt to claim their new passport in person, they may do so  and they need to bring  their old passport for cancellation.

ADDITIONAL REQUIREMENTS

In some cases,  additional requirement may be necessary.

In case the applicant is unable to claim his/her passport, immediate family members* are allowed to claim the passport in behalf of the applicant
  • Authorization letter (must mention affiliation with applicant),
  • Original DFA issued receipt,
  • Valid ID (original and photocopy) of applicant and representative
* Spouses, parents, and brothers/sisters, or children of legal age
In case a person other than an immediate family member will claim the applicant’s passport
  • Notarized original Special Power of Attorney (SPA) authorizing the representative to pick the applicant’s passport
  • Valid IDs (original and photocopy) of applicant and representative indicated in the Special Power of Attorney
For minor applicants
  • If passport will be claimed by mother, she must present original DFA- issued receipt and valid ID.
  • If passport will be claimed by father, claimant must present original DFA issued receipt, valid ID and PSA documents indicating proof of parental authority over the minor

Yes. If you reside in Germany and live near the border of another European country, you may apply at the Philippine Embassy or Consulate that has jurisdiction over that area. Please contact the appropriate Philippine Embassy or Consulate to confirm their requirements and appointment procedures before applying.

The processing time depends on the volume of applicants at the day and time of your visit. On average, the application takes about 20 minutes, and the fee is €60.00.

Yes, an appointment at the Consulate is needed for passport renewal. Clients may book an appointment through the passport.gov.ph website.

After completing the biometric data capture at the Consulate, it typically takes about 4 to 6 weeks for the new passport to arrive. All passports are centrally printed in Manila. However, it is best to regularly check the Consulate’s  website for announcements on possible delays.

When the new passport arrives, the applicant must bring the old passport to the Consulate for cancellation. The old passport will be cancelled and returned to the applicant together with the new passport.

Yes, personal appearance is required for all passport appointments for capturing of your biometrics. 

Applicants may renew their passports 9 months before its date of expiry.

No, there is no additional fee for renewing expired passports.

Once the Report of Marriage has been duly filed, she may then apply for a new passport reflecting her married name.

Yes, your child is entitled to a Philippine passport, provided that your wife was still a Filipino citizen at the time of the child’s birth. The child’s birth must be reported to the Philippine authorities in Manila. Click link for the form and information on the Report of Birth (ROB).

Once the Report of Birth has been duly filed, your child can apply for a Philippine passport.

Yes, it is possible and more efficient to file the Report of Birth (ROB) concurrently with the passport application, or to combine the Report of Marriage with a passport renewal, provided that all required documents are complete. This allows applicants to accomplish both procedures in a single visit to the Consulate.
