IMPORTANT REMINDERS (Please read carefully):

  1. This appointment system is for those applying for CIVIL REGISTRY, which include the following consular services:
    • Report of Birth
    • Report of Marriage
    • Report of Death
    • Correction of Clerical Error in Philippine Statistics Authority (PSA) Birth Certificates
  2. When booking an appointment, you will be required to provide your full name, a valid email address, and the type of transaction. You may also, at your discretion, provide your contact number and sex.
