APPOINTMENTS FOR CIVIL REGISTRY SERVICES

IMPORTANT REMINDERS (Please read carefully):

  1. This appointment system is for those applying for CIVIL REGISTRY, which include the following consular services:
    • Report of Birth
    • Report of Marriage
    • Report of Death
    • Correction of Clerical Error in Philippine Statistics Authority (PSA) Birth Certificates
  2. When booking an appointment, you will be required to provide your complete name, valid email address, phone number and type of transaction.
  3. Upon completing your online booking, please check your email immediately and follow the instructions in the email in order to CONFIRM your appointment. The appointment will not be reserved if you do not confirm within one (1) hour.
