The Philippine Consulate General issues consular mortuary certificates for human remains that will be transported to the Philippines.
Requirements for the shipment of human remains depend on whether the body was embalmed or cremated. and what citizenship the deceased was carrying at the time of their passing.
Applications for Consular Mortuary Certificates must go through online pre-assessment prior to submission to the Consulate in person or by post.
REQUIRED SUPPORTING DOCUMENTS:
* If the deceased was a Filipino citizen at the time of their passing, please comply with requirements listed on items 1 to 10. **If the deceased was NOT a Filipino citizen at the time of their passing, please accomplish only applicable requirements listed on items 2 to 10.
1. Four (4) original copies of the duly accomplished Report of Death Form;
2. Original and five (5) photocopies of the Death Certificate/Registry (Formula C Auszug aus dem Sterbeeintrag) issued by the Local Civil Registrar (Standesamt) or similar authority showing the name, age, nationality, date of death and place of death of the deceased person;
3. Medical Report of Death (Leichenschauschein), stating the cause of death as certified by the attending physician, and accompanied by a certified English translation;
4. Certificate of Non-Contagious Disease issued by the proper health authority (e.g., Gesundheitsamt) stating that the deceased did not die of a contagious disease or that the remains to be shipped did not originate from a place contaminated by a contagious disease, in English or accompanied by a certified English translation
5. Copy of the passport or Personalausweis of the deceased
6. Additional requirements if CREMATED:
- Notarized Certificate of Cremation (Einäscherungs-bescheinigung) issued by the crematorium, in English or accompanied by a certified English translation, stating that:
- The body was properly cremated in accordance with internationally accepted practices;
- The ashes are in a sanitary condition for overseas shipment, consistent with existing standards; and
- The urn has been hermetically sealed and contains only the remains of the deceased.
- Copy of the planned/indicative flight details and Airway Bill if remains will be sent to the Philippines as unaccompanied cargo, OR
- Copy of the planned/indicative flight details and passport of the person who will bring the urn to the Philippines
7. Additional requirements if EMBALMED:
- Notarized Mortuary Certificate (Bescheinigung) issued by the Mortuary/ Funeral Director + Embalming Certificate (Bescheinigung der Einbalsameirung von Verstrobenen), in English or accompanied by a certified English translation, stating that:
- The body was properly embalmed in accordance with internationally accepted practices;
- The body is in a sanitary condition for overseas shipment, consistent with existing standards; and
- The coffin/casket has been hermetically sealed and contains only the remains of the deceased.
- Corpse Transit Permit (Liechenpass)
- Copy of the planned/indicative flight details and Airway Bill
8. Accomplished Letter Request Form for issuance of Mortuary Certificate
9. Self-addressed return envelope with EUR 4,45 postage stamps for registered mail (Einschreiben) if processed documents will not be picked up
10. Documentation Fees:
- Fee for the issuance of the Report of Death (for deceased Filipinos): EUR25,00
- Additional fee for Affidavit if applying for Delayed Report of Death: EUR25,00 (only for Filipino nationals)
- Fee for the issuance of the Consular Mortuary Certificate: EUR25,00
SUBMISSION PROCEDURE
If sending the documents by post, please follow the below steps:
- STEP 1: Email your filled up but unsigned ROD application form and scans of your complete supporting documents to civilregistry.frankfurtpcg@gmail.com if the deceased was a Filipino citizen at the time of their passing, or to legalization.frankfurtpcg@gmail.com if the deceased was not a Filipino citizen for pre-assessment.
- STEP 2 (applicable only if the deceased is a Filipino): Once the Consulate confirms by email that your ROD application form and Affidavit of Delayed Registration is correct, have one ROD application form notarized and any accompanying Affidavits (if applicable) notarized AND pre-authenticated by the relevant Landgericht.
- STEP 3: Once the Consulate confirms by email that your documents are ready for submission, assemble your packet to include the relevant forms and all of the supporting documents you submitted during the pre-assessment stage, along with your proof of payment and self-addressed prepaid return envelope.
- STEP 4: Mail your documents to the Consulate at this address:

B. If submitting the documents in person, please follow the below steps:
- STEP 1: Email your filled up but unsigned ROD application form and scans of your complete supporting documents to civilregistry.frankfurtpcg@gmail.com if the deceased was a Filipino citizen at the time of their passing, or to legalization.frankfurtpcg@gmail.com if the deceased was not a Filipino citizen for pre-assessment.
- STEP 2: Once the Consulate confirms by email that your documents are ready for submission, book your appointment here and prepare the forms and all supporting documents that you submitted during the pre-assessment stage, along with a self-addressed return envelope with EUR 4,45 postage that will be used to send back your documents if you do not plan to claim them personally.
- STEP 3: Arrive at the Consulate for your scheduled appointment on time, with your complete documents, to file your application.
REMINDERS
- Applications may only be filed in person or by mail AFTER the applicant has had their requirements pre-assessed by sending scans of their complete supporting documents to civilregistry.frankfurtpcg@gmail.com.
- All documents must be in English or accompanied by an official English translation.
- The Consulate will NOT process an application unless all requirements are complete, with the required translations and notarizations.
- Applicants are reminded NOT to confirm any flight details for the remains until after the Consular Mortuary Certificate has been issued.
- The Consulate may require additional documentation from the applicant depending on the circumstances of the application.
